MivaPay CVV Required field should be on

I'm pretty sure I had this configured correctly except for the Payment card On/Off where it appears the default is off.

In MivaPay Settings and the Checkout CVV field is set to "REQUIRED" the field display in the Payment Card Window for the pay card group of cards should be turned on by default. So far it appears that it's off by default.
