MailChimp Form Integration with Wordpress dynamic header / footer

Does anyone know what files are need to get the Mailchimp form to work in a global footer that is also being pulled in dynamically to Wordpress?

I've tried adding the CSS & JS as well as the new mailchimp_tracker script but it's not working:
/mm5/css/00000001/mailchimp.css
/mm5/scripts/00000001/mailchimp.js

Where is the wordpress blog located at? on a subdomain? The JS does an ajax call back to the miva store so if it is being done from another domain it won't work. You may have to hide the form on the wordpress site if that is the case.

It's on the same domain just in a subdirectory /blog/. Are there relative URLs in the ajax call?
