need suggestion - catalog subscription for Intl customers

My client prints four catalogs a year. Legitimate requests from
international customers for catalogs are spendy to send out. They want
a solution like this...

All US customers can get a free catalog whether they buy any product or
not.

International customers must pay $7 for their printed catalogs if they
don't buy product.

International customers get the catalogs for FREE if they buy any
product.

I'm looking for something like:

Add 'Printed Catalog for Intl Customers' product to cart.
if its the only product in the cart, price is $7
if there are other products in the cart, price is $0

Any mostly elegant suggestions?

One of my wholesale clients has a 48 page full color catalog, also updated
and printed 4 times a year. We have a password protected PDF version of the
catalog online for people who need immediate access to the catalog and
international customers. We also post the catalog without pricing. The
people who do the catalog layout provide me with the PDF files.
