Levels Newsletter Signup Form and MailChimp

I am switching to Mailchimp and I am trying to integrate the mailchimp text box with the Levels designed Newsletter Signup form. Anyone have some sample code I can try. I try to merge the two but it doesn't seem to trigger properly. I got the checkbox working on the OCST page, so I am sure I'm close, just missing something.

Since you are dealing with two form elements, I would recommend hiding the MailChimp form on the page and add some JavaScript which would copy the email address entered into the contact form to the hidden MailChimp form and the, on click of the contact form submit, submit the MailChimp form first and then the contact us form.

Thank you for the reply. I was hoping that someone already had some code I could borrow as JavaScript and I mix like oil and water.
