Contact form with upload field for jpg, gif, png, or pdf file

I need to create a contact form for a site (using the Levels ReadyTheme) that has the ability to upload a jpg, gif, png, or pdf file. Very similar to this here.

Hi Leslie,

Generally, that would be handled by a custom form processor, usually coded in PHP. There are many options for them out there. I'm not sure if any of the developers on the board have a "drop-in" solution, but you never know.

Not drop in, but just implemented this on a site...has Drag and Drop interface as a bonus. I'll send you the details off list.
