Customized Contact Form

Working on a customized contact form where customer can input information for us to generate a quote.. I used the contact_form from the CTUS page, and made a new theme component/content section called custom_form. I also generated a new CTFM page for custom forms. I went in to the back end and found the Extensions scripts and added one for the custom form... Not quite sure if I've done more than necessary, or missing a simple step, but my submit button is greyed out. Thanks for taking the time!
