How to let customer upload images to website

Hi
Can any one help

I need to let customers upload an image to my miva 5.5 website from the front end.

For example, If a customer wanted to buy a T shirt with their name on,
they would need to be able to choose the size, colour and upload and image to be printed on that t shirt, probably via an Open dialog for files on their local computer.

oh yes and some way of downloading it back to my computer

We use the File Upload Attribute module by Kent Multer, Magic Metal Productions. This module does exactly what you're looking for and it works great for us. You'll find him here in the forums.
