How to Add an Upcharge to Order

I can't figure out how to add an additional flat fee at checkout for an item that requires a liftgate. ( Regardless of the quantity ordered)

I would like it to display as a separate line item.
If I add it to the product's Price Table Shipping Charges, or as an attribute option, the fee gets multiplied by the quantity ordered.

Everything I've looked at in the shipping options seems to be based on $ amount or weight, which won't work either.
Am I overlooking something obvious? Thanks.

Check out the Miscellaneous Fee module at Emporium Plus. We use it in our stores for all sorts of things.
