general entity "Screen" not defined and no default entity

I have this warning when running html validator and I'm trying to figure out how to fix it. I have been reading article and trying to find the answer, but nothing I try works. Here is the warning:

general entity "Screen" not defined and no default entity.…_ID=5e41729f78fa99a6337750a7e2f7d8f7&Screen=LOGN&Order=0&Store_Code=FTS"><img

I get the same warning for the &Order that is after and all over the site.

What is the fix for this?

you need to replace the & sign with the appropriate HTML &amp; code or simply ignore it.
