Doubled Shipping?

I just played around with you cart. It's repeatable and happens when you have a different bill to / ship to. Not sure what the fix is.

I am looking to find out if anyone else is experiencing double shipping charges on orders. I wrote to support a couple of weeks ago and was told this is a rare event but I've had 13 orders in the past 2 weeks where the customer was charged twice for their shipping. We use Price Table Based Shipping in our store.

I am trying to get to the bottom of this as my larger concern is how many customers might have not completed their purchase when they saw duplicate shipping charges during checkout. Anyone else experiencing this?
