Basically, you create a custom field for each of the old Addendum questions; then you put Read_XXX and Write_XXX tags on the page templates to save and display the fields (where XXX may be Basket or Order, depending on the page). The devil is in the details, of course ... I've done this work for other clients; you can drop me a line by email if you need more help.Kent Multer

You should also be able to add "required" and even "fees" using native SMT coding...but yea, i think for store owners, someone (hint, hint) should write a tutorial.Bruce Golub
